Mansfield Ohio Cornerstone Caregiving Patient Nearly Dies Under Care of Drug Addict, Felon & Mentally Ill Caretaker

MANSFIELD, OHIO – Cornerstone Caregiving, an agency providing non-medical in-home support for seniors, employed a female caregiver with a documented criminal history involving felony theft and drug-related offenses. On February 3, 2026, a patient with dementia under her supervision wandered from the residence into freezing temperatures. Mansfield police located the individual outdoors and alerted emergency services; the patient was subsequently hospitalized with severe hypothermia, recording a core body temperature of hypothermic degree.  Although the caregiver told police she fell asleep for approximately 40 minutes, this timeline is inconsistent with the patient’s body temperature, which indicates a lengthier period of time than was stated to police.

On the day of the incident, an independent investigation revealed that the caregiver conducted dozens of alarming Google searches, several of which occurred while she was at the home of the patient hospitalized due to her negligence. These searches included queries regarding her abusing her children, them inflicting self-harm as a result, and potential prosecution for allowing her patient to wander. Additional searches involved the local Mansfield hospital, the police, and the Operating Director, Noah Bolen, regarding a personal dispute over alleged defamation.

Cornerstone Caregiving Terminated Employee in Regards to Hospitalized Client says Jacqueline Vanek, Assistant Operating Director

This is not the first time this caregiver has caused issues with senior citizens and patients. In 2024, while employed by Amedisys Home Healthcare, she was threatened with elder abuse charges by administration for neglecting a patient under her care. The patient suffered severe bedsores from head to toe due to neglect, including being left in the same position for upwards of 12 hours without medication, rotation, or basic hygiene, often left in their own feces. The patient (her own grandmother) passed away just hours after her removal from care, she did not find employment again in this occupational field until hiring on with Cornerstone Caregiving.

Months prior to this incident, in 2025, Cornerstone Caregiving was formally advised of the severe risks posed to clients, the company, and herself. Management was notified that she had recently been released from a psychiatric ward, that her behavior was deteriorating rather than improving, and that she had stopped taking her medication.  The company was informed that she posed a risk to that of herself, her patients and her company and the individual expressed sincere concern for the wellbeing of all parties involved.  Furthermore, Cornerstone was informed that she had been terminated as a patient by her mental health provider, Hope419, and was alerted to previous allegations of abuse during her time at Amedisys – all of which fell on deaf ears.

ADDITIONAL INFORMATION: From her patient’s home, the caretaker also posted several TikToks that inadvertently showcase the living environment and personal effects, such as bookshelves, in the background.  In addition to the aforementioned searches, additional search history results reveal a disturbing pattern: she has researched acquiring controlled substances like Xanax via the dark web and screened various medications to determine their recreational potential, acquiring fentanyl testing strips and engaged in conversations making light of her drinking and partying habit and it’s effects on her performance while taking care of others.

FAMIL MEMBER STATEMENT ON CARETAKER:  “She shouldn’t be taking care of anyone right now other than herself.  She needs to focus on herself, get on the proper medications, mental health treatment and then she can go about taking care of other people.  She is currently in no condition to take care of anyone, not patients, not her kids, nobody, this is the worst that I have seen her in the entire time I’ve ever known her.”

Search history results of the Cornerstone Caregiving employee in regards to her client wandering outside into frigid temperatures until body temperature reached that of 68 degrees and those pertaining to the abuse of her children as well as attempts to obtain controlled substances.  The day of.

CONVERSATIONS

The following conversations are between the Cornerstone Caretaking caregiver and that of other individuals.  In one instance she makes mention of how she is hungover at the job site (caretaking), and in other’s she makes light of the situation in regards to almost killing her own patient that she left wandering outside to the point that they had hypothermia and had to be hospitalized.

Zero accountability with a lack of genuine compassion.

SCUMBAGGED Tip:  Cornerstone Caregiving should be ashamed of itself.  Not only were they informed before their client needed to be hospitalized and nearly lost their life, but they have intentionally and willfully ignored information (for a period of time at least) provided and failed to respond for comment when we reached out.  We strongly urge everyone that uses this company to perform their own background checks on individuals as well as that of their social networking accounts prior to allowing them into the confines of their homes to take care of their loved one(s).  It is imperative that this is done because this is apparently a company that believes in profits over protecting that of their clients.

Cornerstone Caregiving was reached out to for comment by Scumbagged.com regarding this ongoing situation and they have yet to respond via email.
